How do I rent a private mailbox?
1.	Fill Out a USPS 1583 Form.

	Please note that if you are mailing your forms in, the 1583 form must be notarized.
2.	Bring in 2 current forms of I.D. Please note that Credit Cards, Social Security Cards, and Birth Certificates are all expressly prohibited as ID.
	Acceptable Forms of Identification:
	Driver's License State ID Armed Forces ID Government ID University ID Recognized Corporate ID Passport	Alien Registration Card Current Lease, Mortgage or Deed Voter Registration Card Utility Bill Home or Vehicle Insurance Policy Vehicle Registration

3.	Fill out our Mail Receiving Agency Form.

4.	Set up custom forwarding if applicable.

Personal Mailbox:

Fill in the following boxes on the 1583 form: 1, 2, 3a (please call ahead for your mailbox # so we can reserve it for you), 6, 7a, 7b, 7c, 7d, 7e, 8a, and 8b.

Business Mailbox:

Still fill in all parts needed for a personal mailbox plus the following: 9, 10a, 10b, 10c, 10d, 11, 12, and 13.

When you sign box #s 5 and 15 the US Postal Service requires the form to be notarized.

Please make sure to mail in original forms. Mailbox will not be rented until we receive forms and copies of identification.
